3.* Federal Criminal History Record Information - (CHRI) - FBI Report
To begin pre-enrollment for your FBI fingerprint, click the above link and enter the requested information. Please select a photo identification document from the list provided. You must produce that same ID at the fingerprinting site. The five closest fingerprinting sites will be listed. Select location, date, and time for your fingerprinting appointment. The fee is $22.60 payable by major credit cards.

This form is required for employees and independent contractors. You are required to complete a form for your current employer, any schools which you worked or all employers where you worked with children. The district will send forms to your current and former employers. If you have had these forms completed by your current/former employers the district will verify them.

All information must be within 1 year of your hire date.
* As of December 2014, Pennsylvania Act 153 of 2014 requires that school district employees obtain new clearances every 60 months. Once hired by the district, you will be required to obtain new clearances every 60 months from the date of the oldest clearance.
